Job Description:
HealthONE, a cornerstone of HCA Healthcare, is seeking a
dedicated and compassionate RN Case Manager PRN to enhance our
patient management staff in Aurora, CO. Our facility is committed
to providing superior medical services, specializing in
comprehensive, patient-centered care across various medical
specialties. In this role, you will be responsible for assessing
patient needs, planning and coordinating treatments, and overseeing
the entire case management process to ensure the highest quality of
care is maintained. You will play a pivotal role in advocating for
patients and their families, helping them navigate the complexities
of the healthcare system while upholding our mission of
compassionate service. Conduct comprehensive patient assessments to
determine necessary care and treatment plan. Coordinate healthcare
services across multiple departments to ensure continuity of care.
Act as an advocate for patients and their families, providing
guidance and support through medical processes and decision-making.
Monitor patient progress and adjust care plans as needed. Work
collaboratively with healthcare professionals to ensure patient
needs are met with the highest standards of medical care. Maintain
accurate patient records and ensure compliance with all healthcare
regulations. Provide education and resources to patients and
families to aid in recovery and to support health maintenance and
prevention. Strong clinical skills with an emphasis on case
management. Excellent communication and interpersonal skills.
Ability to work independently and as part of a team. Strong
organizational skills and attention to detail. Proficiency in
electronic medical record systems. Medical Insurance Dental
Insurance Vision Insurance Life Insurance 401k Current Registered
Nurse (RN) license in the state of Colorado. Certification in Case
Management (CCM) preferred but not required.
