This pivotal role focuses on facilitating smooth transitions for patients moving between acute care settings and additional supportive environments, ensuring continuity of care and optimizing patient outcomes. As part of our healthcare system, you will collaborate with multidisciplinary teams across various hospitals and outpatient facilities to coordinate effective care plans, adapt to patient needs, and uphold the highest standards of care. Assess patient's medical condition and history to appropriately plan for care transitions. Coordinate with healthcare professionals from multiple departments to ensure a seamless transition for patients. Develop and implement transition plans that include patient education and involvement. Monitor patient outcomes post-transition and adjust care plans as needed. Participate in continuous education to stay informed about best practices and innovations in care transitions. Strong clinical skills and knowledge of acute care Excellent communication and interpersonal skills Capability to work effectively in a multidisciplinary team Proficiency in healthcare software and electronic health records Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Valid RN license in the state of Colorado Certification in Care Transition Nursing is preferred
