Job Description:
UCHealth is seeking a compassionate and dedicated Nurse
Navigator to join our dynamic team, providing top-tier
patient-centered care across our various Colorado facilities. As a
Nurse Navigator, you will play a pivotal role in coordinating
comprehensive healthcare services to ensure a seamless and
supportive experience for our patients. This position requires
excellent interpersonal and organizational skills, as well as a
deep commitment to patient health advocacy and education. Serve as
the primary point of contact for patients, guiding them through the
healthcare system. Assist in scheduling appointments and
coordinating with various healthcare providers. Educate patients
and their families about treatment plans and healthcare resources.
Monitor patient progress and adjust care plans as necessary in
consultation with healthcare teams. Collaborate with
interdisciplinary teams to provide holistic care. Document patient
interactions, treatments, and outcomes accurately in the patient
management system. Strong interpersonal and communication skills.
Ability to work efficiently in a high-pressure environment.
Exceptional organizational skills and attention to detail.
Proficient in using medical software and patient management
systems. Medical Insurance Dental Insurance Vision Insurance Life
Insurance 401k Valid Registered Nurse (RN) license. Certification
in Healthcare Navigation is preferred but not required.
