winners. Responsibilities Manage patient care plans and coordinate
services for optimal outcomes. Conduct assessments to identify
patient needs and develop care strategies. Achieve performance
targets and contribute to team success. Collaborate with healthcare
teams to ensure comprehensive patient support. Participate in
professional development and training opportunities. Engage in
continuous learning to enhance clinical skills and knowledge.
Monitor patient progress and adjust care plans as necessary.
Educate patients and families on care plans and health management.
Document patient interactions and maintain accurate records.
Utilize data to track performance and improve service delivery.
Participate in quality improvement initiatives to enhance patient
care. Provide support and guidance to junior staff and new team
members. Experience/Qualifications Valid RN license in Colorado
with case management experience. Commitment to continuous
professional development and training. Strong assessment skills to
identify patient needs effectively. Proven ability to achieve
performance targets in a healthcare setting. Experience
collaborating with multidisciplinary healthcare teams. Strong
documentation skills for maintaining accurate patient records.
Experience using data to track performance and drive improvements.
Ability to educate patients and families on care management.
Participation in quality improvement initiatives for patient care.
