Job Description:
We are seeking an experienced Transitional Care Management Nurse
to support patients transitioning from inpatient to home care. No
weekends or holidays, with a $1,000 sign-on bonus ($500 after
probation, $500 after 6 months).

Work as part of a team to improve patient outcomes in a supportive
environment with no weekend shifts.
